Laureate Medical Group participates with most insurance plans. If you are not insured by a plan in which we participate, payment in full is
expected at the time of your visit.
Understanding your insurance benefits is your responsibility. Therefore, please be sure that you verify that our physicians are in your network, what your out-of-network benefits are, and what your insurance covers. If you have questions about your coverage, please contact your insurance company with any
questions before your appointment.
Laureate Medical Group participates with most of the major health insurance plans in the State of Georgia. Be sure to ask about the participation status of your health insurance plan when scheduling appointments and at each office visit.
The patient is responsible for letting us know of changes in insurance and other information prior to the appointment. You may easily update your information with us by logging on to "MyLaureate."
Co-payments, annual deductibles and co-insurance amounts are the responsibility of the patient and are due at the time of service. These payments will be collected at the time of your appointment. Failure to make the required payment will result in an administrative charge to your account.
For patients without insurance, full payment is due at the time of service. Payment can be made by cash, check, debit card or credit card.
