Patient Information

Phone Calls

Due to the high volume of calls, often you will reach our voice mail system. We assure you that all voice mailboxes are checked regularly and calls are returned promptly and appropriately during business hours.

Effective January 5, 2015, the phone lines for all offices will be open to accept calls from 8:00 am - 4:00pm.  

To ensure that your needs are met expeditiously, please leave a complete message including your name, date of birth, your provider, your reason for calling, and a contact number so we can return your call. Please note that any medical questions must be forwarded to the provider for review. Since our provider may be busy with patients during the day, it may take some time to reply to your question.

The best time to call is Tuesday through Thursday between 10 a.m. and 2 p.m.

Your Medical Records

Secure and reliable access to our patients' medical records is available through our on-line electronic medical records system.
The relationship between the doctor and patient is private and any information exchanged will be held in the strictest of confidence. Confidential medical information will be released only with the express consent and authorization of the patient.

HMO/POS Referrals

For HMO/POS referral requests, please call the nurse at the office you visit. Referrals take up to 72 hours for processing by most insurance companies, so please plan accordingly when making your requests.

To complete your request, we will need the following information:
  • Patient's name and date of birth 
  • Caller's name and phone number 
  • Patient insurance and identification number 
  • Name of the specialist or facility 
  • Reason for visit and date of appointment
  • FAX number

We will submit all the required information to the insurance company, but it is up to your insurance company to decide if the referral is approved.

Appointment Cancellations

In order to provide all our patients with the best care possible, we ask that you make every effort to keep your scheduled appointments and arrive at least 15 minutes early for your appointment.

If you need to reschedule or cancel an appointment, we require a minimum cancellation notice of 24 hours. Please remember that confirmation reminders from us are only a courtesy. Our failure to confirm your visit does not relieve you of your responsibility to cancel your appointment.

A $50 fee may be charged when a patient fails to provide us with at least 24 hours’ notice of cancellation, or is a “No Show” for the following services:

  • All diabetic education
  • Nutritional counseling
  • Follow-up visits

A $100 fee may be charged when a patient fails to provide us with at least 24 hours’ notice of cancellation, or is a “No Show” for the following services:

  • All new patient visits
  • Consults
  • All scheduled procedures
  • Existing patient physicals

Failure to Pay at Time of Service

All fees that are your responsibility are due at the time of your appointment. These include any co-payments, co-insurance, deductibles, or any other charges not covered by your insurance provider.
